General Registration Information • Your child will not be considered to be enrolled in The Chrysalis School until the Registration Fee and last month’s tuition has been paid. • Your child will not be considered to be enrolled in The Chrysalis School until all paper work has been received. All paperwork must be completed and signed. • Please attach your child’s current shot record to the Health Form. The Shot Record is not considered a Statement of Health. • You may attach your physician’s own statement of health form to the Health Form. • We are not able to accept students that require an Epipen for severe allergies. Registration Forms The registration and enrollment forms must be ready to turn in at registration. Please note: The health form needs to have a doctor's signature.
